When to call

Some examples of things a Care Manager helps with:

When you need to know about resources for yourself or a loved one.  For example, I am familiar with providers of in-home care as well as long term care facilities in the area.

 

When you have a concern that you want to discuss confidentially with a professional.

 

When you are unsure what you need and require an assessment, information or education on a topic of psychosocial concern.

 

When you need support as a caregiver.

 

When you need to find a reliable repairman.

 

When you need a ride to medical or other appointments or to church.

 

When you are going through a transition or difficult time in your life.

 

When you have questions about your medical insurance.

 

When you have a need that might be met by a volunteer.
