The position of care manager came about following a study of the needs of the aging and frail members of the church conducted by the “Advocates for Generations of the Elderly” task force at BMPC. From that study, a recommendation was made to add a professional staff person with skills in social work and knowledge of resources available to help people live their lives more fully.
Care Management is a ministry provided by the church for members, their families and loved ones when health, aging or life circumstances raise social or psychological concerns. It is practical help to assist you in defining a way to live well.
As a member of the pastoral team, the Care Manager works closely with the pastors, Middleton Center, Congregational Care Council, the Senior Adult Council, and the Parish Nurse.
